One-on-One Coaching Service Agreement

This Service Agreement ("Agreement") is made and entered into on the date of purchase, by and between:

Client

1. Services Provided

Coach Jasmyn agrees to provide one-on-one coaching sessions to the Client. These sessions will focus on helping the Client achieve their personal and professional goals through tailored advice, strategies, and support.

2. Session Details

  • Number of Sessions: 4
  • Duration of Each Session: 30 Minutes
  • Frequency of Sessions: Weekly
  • Mode of Delivery: Virtual
  • Session Fee: USD 750.00

3. Payment Terms

  • Payment Method: Payments can be made via Credit Card and Debit Card.
  • Payment Schedule: Payment is due Monthly.

4. Rescheduling and Cancellations

  • Rescheduling: Sessions can be rescheduled with at least 24 hours' notice. To reschedule, contact Coach Jasmyn or reschedule through the platform.
  • Cancellations: Make-up sessions are not permitted for canceled sessions unless it is within the same week. 
  • Missed Sessions: If the Client misses a session without prior notice, the session will be forfeited.

5. Confidentiality

All information shared during coaching sessions is confidential and will not be disclosed to third parties without the Client's consent, except as required by law.

6. Commitment

The Client agrees to be committed and fully engaged in the coaching process. This includes attending sessions on time, being open and honest, and completing any assignments or action steps agreed upon during sessions.

7. Termination

Either party may terminate this Agreement with written notice. If the Client wishes to terminate the Agreement, any unused session fees will not be refunded.

8. Disclaimer

Coaching is not therapy or counseling. Coach Jasmyn does not diagnose or treat mental health conditions. The Client understands that coaching is a professional relationship designed to support personal and professional growth and development.

9. Limitation of Liability

Coach Jasmyn will not be liable for any indirect, incidental, special, or consequential damages arising out of or in connection with the services provided.

10. Entire Agreement

This Agreement constitutes the entire agreement between the parties and supersedes all prior agreements, understandings, and representations.

11. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of North Carolina.
